Service Description
Select this option if you want to self-pay for medical weight loss visits that are not covered by insurance. Routine follow-ups are required to check response to weight loss treatment. Ideally, you should have a scale available to measure your weight, and a blood pressure cuff to keep track of blood pressure and heart rate.
Cancellation Policy
APPOINTMENT CANCELLATION, RESCHEDULING, AND NO-SHOW POLICY Cancellation or Rescheduling With at Least 24 Hours' Notice There is no charge to cancel or reschedule an appointment when at least 24 hours' notice is provided. Late Cancellation or No-Show Except where prohibited by law or the patient's health insurance program, an appointment canceled by the patient with less than 24 hours' notice, or an appointment for which the patient does not attend without prior notice, may be subject to a $25 late-cancellation/no-show fee. The fee may be charged to the credit or debit card on file. Practice or Provider-Initiated Cancellation or Rescheduling Occasionally, the provider may experience an emergency, illness, unexpected clinical obligation, technical issue, or other unforeseen circumstance that requires 9 Lives Wellness, LLC to cancel or reschedule an appointment, including with less than 24 hours' notice. Patients will never be charged a cancellation, rescheduling, or other appointment-related fee when 9 Lives Wellness, LLC or the provider initiates the cancellation or schedule change. The practice will make reasonable efforts to notify the patient as soon as possible and offer an opportunity to reschedule the appointment. Repeated No-Shows Except where prohibited by law or the patient's health insurance program, a patient who has more than three no-call/no-show appointments within a six-month period may be required to provide a $50 appointment deposit before scheduling future appointments. The $50 deposit will be applied toward amounts appropriately due for the scheduled appointment. If the patient attends the appointment or cancels or reschedules in accordance with this policy, the deposit will not be forfeited. If the patient fails to attend the scheduled appointment without providing notice, the $50 deposit will be non-refundable. Repeated missed appointments may result in limitations on future scheduling or termination of the patient-practice relationship when permitted by applicable law and after appropriate notice.
